Issues loading DICOM volume into Slicer

Hi,

I'm new to all of this so please bear with me.  I'm trying to load an ultrasound volume into slicer by first turning it into DICOM using dicomwrite.m in matlab.  I created the .dcm and set the necessary metadata (or at least I think I have).  It can be found here:

I get the error: Could not load: Unnamed Series as a Scalar Volume.

The error log reads:

If I only save one frame/slice of the ultrasound it loads successfully, so it's the volume that's an issue.  I expect I am missing something that has to do with the spacing or geometry when creating the dicom that will allow Slicer to read it.  Any help is appreciated!
